Description of Services: Program assists people living with dementia and their families make informed choices for better quality of life

Services may include:
  • assistance with identifying and accessing information, supports and services that meets needs
  • facilitates connections to education, programs and services within the organization as well as other health care and community service providers
  • one-on-one counselling
  • caregiver support groups
  • support groups for persons newly diagnosed
  • referral to community resource by phone, drop in, or home visit
