Description of Services: Patient-centred support for people living with prediabetes, type 1 diabetes, type 2 diabetes, diabetes and pregnancy, and steroid induced diabetes

Services include:
  • one-on-one education on monitoring blood sugar levels, healthy eating, how to administer insulin, etc.
  • Registered Nurses (certified diabetes educators) and registered dietitians (certified diabetes educator) on staff. 
  • insulin pump certified educators
  • endocrinology clinics

Application: Referral from a Physician or Nurse Practitioner * referral form online or call for information
