Description des services: Not-for-profit organization that facilitates and supports those who want to live independently at home * localized programs and services that are staffed by a team of healthcare workers and community volunteers to meet needs of the community 

Services and programs available:

Adult Day Program - Social and recreational activities for adults with Alzheimer Disease or related dementias. Respite and support also available for caregivers.

Seniors Maintaining Active Roles Together (SMART) - Exercise classes for members that are older than 55 years that assist in improving strength and flexibility. Virtual options are available.

In-Home Adult Respite - PSW Support (set fee) for clients who have Dementia * caregiver relief/support also available

Home at Last - Free service for seniors 55 years and older to bring home from hospital * assists with medication and grocery pickup

Frais: Some programs are free *Set fee for respite care
Admissibilité / population desservie: Older adults 55 years and older * youth and adults who are recipients of Ontario Works or Ontario Disability Support Program, or discharged from hospital after an overnight stay for a condition requiring physiotherapy
